Hardeman Healthy Outreach (H2O) for Healthy Weight Farm to School Survey for Parents

Dear Parent/Guardian of a Hardeman County student:

The University of Tennessee Extension team is asking for your help in completing an anonymous online survey about your family's food preferences. The results will be used to develop Farm to School initiatives in your county. The University of Tennessee Extension receives funding for the Hardeman County Farm to School program from the Centers for Disease Control and Prevention (CDC).

All responses are anonymous and will remain confidential. You must be age 18 or older to take the survey. By completing the survey, you will be eligible to enter a drawing for a $5 Walmart gift card. Responses are due by April 30, 2021.
